Please complete this form and e-mail to the school nutrition manager Luetitta Whitlock to receive fee waivers for ACT/SAT and/or college applications  The school nutrition manager MUST verify your status.  Once the form is verified by Mrs. Whitlock, e-mail it to your counselor in order to receive your fee waiver.
