Clinic Aide*When bringing in medication please fill out the proper form/forms. Please make sure the medication is in the original container, age appropriate and not expired. If medication is filled by a pharmacy, please make sure the pharmacy label is attached and is legible.*All life-saving medications will need an action plan form along with either a carry form (which both require a doctor's signature) or a medication authorization form (which doesn't require a doctor's signature) for medication to be housed in the clinic.*No 24-hour medication unless your doctor has ordered you to take it at a certain time. Documentation is required. These medications can be given at home.*If medication requires to be cut, that will need to done either by the parent or the pharmacy.*Other than approved life-saving medications, no student should be carrying any medication, including cough drops.HCS handbook pg.16-IllnessIf a child becomes too ill to remain in class, his/her parents/ guardians will be contacted so the student can be picked up. For the safety of all students and staff, please do not send your child to school if he/she is sick. If a student arrives at school sick, his/her parent/guardian will be contacted to pick up the child. Children should not return to school until they are free from vomiting, diarrhea, and/or fever free, without the use of medication, for at least 24 hours. Regarding fever, a child with a fever (oral temp. of 99.5 degrees or higher with other symptoms of illness present OR temperature of 100.4 degrees or higher, regarding whether other symptoms are present) should stay home until 24 hours without having a fever (without the use of fever-reducing medicine in that 24 hours).* All medication will be thrown out at the end of the school year if not picked up.*All forms are listed below.Click here for the Self Administration of Medication Form- For medication that a student will carry.
