High School Eligibility Requirements
All high school athletics in the State of Georgia are governed by the Georgia High School Association (GHSA). Visit the GHSA website to view eligibility requirements for high school students.
Middle School Eligibility Requirements
All middle school athletics in HCS are governed by the Henry County Board of Education, in conjunction with the guidelines established by the Georgia Department of Education and GHSA.
Athletic Forms
In order for a student-athlete to participate in athletics with Henry County Schools, the following forms need to be completed and on file with the local school Athletic Director:
- Insurance Consent Form
- Concussion Form
- Sudden Cardiac Arrest Form
- GHSA Heat Policy
- GHSA Football Practice Guidelines (Football Only)
- NCAA Form
- Preparticipation Physical History and Evaluation Form (2023)
- Preparticipation Phyical History and Evaluation Form (2023 - Spanish)
- Preparticipation Physical History Supplement - Athletes with Disabilities (2021)
