Change of Name Request

In AccessHCS:

Navigate to “Me tab” – top left corner of page
Under Quick Actions, select “Personal Details”
Select the pencil icon under “Name”, to request a name change
Upload a copy of your updated social security card, reflecting the new name. This is required for HR to process the name change.
After the details are complete, select “Save”. This will be routed to HR for approval.

Change of Address

In AccessHCS:

Login: Open the Access HCS environment and use single sign-on
Navigate to “Me tab” – top left corner of page
Under Quick Actions, select “Contact Info”
“Address” section, click on the pencil, to edit the address details
Country and Home Address (Should be auto-populated)
“When does this address change start?” : Enter the effective date of the new address
Enter the new address
“Zip Code” : enter the zip code and select the zip code displayed, along with the city, county, and state in the list of values
Click “Save”

Public Service Loan Forgiveness (PSLF)

Requesting Your PSLF Form Is Easy

We utilize Verifent to streamline responses and for data security purposes. Visit www.verifent.com, click Get Started, click Login, and click Request PSLF. Follow the steps below.

Note: If you do not have login credentials, please click Register. You will need to create an account.

Step 1: Choose Employer
- Click Choose Your Employer.
- Select your employer needing to review your form and click Save Employer.
- Click Continue.
Step 2: Complete Required Sections
- Section 1: Borrower Information
- Section 2: Borrower Request, Understandings, and Certification
- Section 4: Employer Information
Important: Begin with your most recent start date. Additional Section 4 portions can be requested if there was a break in service.

If you are unsure of the following data elements, they may be left blank for your employer to complete:
1. Employment or Certification Begin/End Date
2. Hours Per Week (Average)
- Additional Message to Employer (if needed)
- Signature: You must sign your name legibly for Federal Student Aid to accept the form.
Step 3: Employer Review and Completion

The form will be sent to your employer to review and complete Section 4.

Once completed, your employer may fax the form directly to Federal Student Aid. If this occurs, you will be notified by email and no further action is required.

To view completed PSLF forms, visit www.verifent.com, click Get Started, and under Employee select Login. Once logged in, you can view completed PSLF forms in the Employee Portal.

If your employer does not fax the form to Federal Student Aid, you will receive an email stating, “You’re Not Done.” Proceed to Step 4.
Step 4: Employee Response
- Click the link in the email and enter your information, including email verification and a two-factor authentication code.
- Locate your request and click View Report.
- All edits made by your employer will be highlighted in red.
- Select Approve and Complete or Disapprove/Cancel.
Approve and Complete
1. Click Approve and Complete.
2. Enter the first five digits of your Social Security number.
3. If you would like the PSLF form to be automatically faxed to Federal Student Aid, move the slider to Yes.
4. Click Approve and Complete to submit.
5. To download or print your completed PSLF form, return to your dashboard, enter the first five digits of your Social Security number, and do not fax the form.
Disapprove or Cancel
1. Click Disapprove/Cancel.
2. Enter the details explaining your disagreement.
3. You will need to submit a new PSLF request.
4. In the Additional Notes section of the new request, clearly explain why you are submitting a new form.
