| Guardianship:
A student must live with a biological parent, or legal guardian who
resides within the boundaries of Henry County, or a state agency with
placement in Henry County. If custody is with any person(s) other
than biological parent(s), a certified copy of the appropriate court
documents will be required at the time of enrollment. Top |
| Withdrawal
Forms: No student will be enrolled without a withdrawal form from
the previous school. Students enrolling without complete records will
be scheduled into regular classes pending receipt of records. Top |
| Summer
Enrollment: Withdrawal forms are not issued for students withdrawing
in the summer. You must provide the last report card from the previous
school year in lieu of withdrawal forms. Top |
| Immunization
Records: Students must provide the school with a copy of immunization
records on a GEORGIA IMMUNIZATION FORM (Form 3231 sample). Form 3231 Requirements Students who do not have this documentation within 30 days of enrollment will be withdrawn. (Kindergarten students must present original form.) Top |
| Eye,
Ear, and Dental Certificate: Students must provide the school
with a copy of an eye, ear, and dental examination of a GEORGIA CERTIFICATE
OF EYE, EAR, AND DENTAL FORM (Form
3300). Students who do not have this documentation within 30 days
of enrollment will be withdrawn. (Kindergarten students must present

| Birth
Record: Students must provide a copy of their birth certificate.

| Contact
Information: Every person registering must furnish the school
with an up-to-date telephone number of a parent or some other responsible
adult who may be contacted in case of necessity. Top |
| Records
Request: When a student enrolls during the school year, a request
for records will be sent to the school previously attended. A student
will not be given credit for achievement at another school if student's
records are not received by Unity Grove Elementary School. Top |
| Special
Programs: If a student has been enrolled in a Special Education
Program (BD, MI, LD, Speech, ETC.) or Gifted Education before coming
to Unity Grove, we must be notified in order for records to be requested
from the school system where the student was previously enrolled.
Students who are enrolled in gifted or special education classes must
have an additional form signed in order to request those records from
the previous school. Top |
| Proof
of Residency: All students must show two proofs of residency. Top |
Proof of Residence: Includes either 1) a copy of a lease or rental
agreement or 2) a property tax statement or home ownership title
(security/warranty deed) and 2 current utility bills (gas, electric,
water, cable, NOT Telephone)
