Kindergarten Registration is May 2 - May 6,
2005During the week of May 2 -
May 6, all Henry County elementary
schools will be conducting pre-registration for students entering kindergarten in the
2005-06 school year. Parents are asked to register their child between
9 a.m. and 3
p.m. at the school the child will attend the first day of the 2005-06 school year.
Attention
of: Parents and/or guardians who will need to complete a Residency
Affidavit. There will be representatives available at the
Henry County Performing Arts Center Lobby from May 2 - May 5,
2005, 8 a.m. to 4:30 p.m. to complete the Proof of Residency
affidavit procedure.
On Friday, May 6th, parents and guardians will need to go to the
Henry County Board of Education Student Services Department located
at 33 N. Zack Hinton Parkway in McDonough to complete the affidavit
residency procedure.

Registration Information and Forms
Kindergarten
students who will attend Dutchtown Elementary, which will open for
the 2005-06 school year, should register at the school in their
current attendance zone.

contact Transportation Services at 770-957-2025.
It is important to bring your child with you to
kindergarten registration so that your child will become familiar with his or her new
school as well as participate in a brief skills assessment. Kindergarten
Questionnaire Form
Georgia law requires that children must be five years old on or before September
1, 2005, to be enrolled in Georgia public school kindergarten programs. To register
children, parents are asked to bring proof of age, such as a birth
certificate;
two proofs of
Henry County residence; an immunization certificate on Georgia Form 3231 (available at
physicians' offices and the county health department).
A student, regardless of grade level, who has never been in a
Georgia public school must also provide certification of eye, ear and
dental examinations. Any child admitted to school without a
certificate must present one within 120 calendar days. The form for
this examination is DHR Form 3300. All DHR forms may be obtained at
the local public health departments or physician offices.
All students entering this
school system are asked to provide to the school the student’s
social security number. No student will be denied enrollment in Henry
County Schools for declining to provide his or her social security
number or for declining to apply for such a number.
A child entering school in Georgia for the first time must be age-appropriately
immunized with all of the required vaccines
(DTP, DTaP, DT, or Td; Hepatitis B; Hib; OPV
or IPV; Measles, Mumps and Rubella or MMR; and Varicella (Chicken Pox). Proof of
Hepatitis B
vaccination is required to enter kindergarten. Parents are being advised to begin the
Hepatitis B vaccinations as soon as possible, because the series of three doses takes a
minimum of four months to complete.
The first day of school for the 2005-06
school year will be August 1. For more information about
