parents/guardians understand some of the requirements for enrolling

Age Requirements
Kindergarten students must be 5 years old on or before September 1. First
grade students must be 6 years old on or before September 1. A copy of the
child’s proof of age must be supplied at the time of enrollment.
Attendance Zone
Students must attend school in the attendance zone where their
parents/legal guardians reside. Students may not attend other schools in
the system except with the permission of the Henry County Board of
Education.

Ear, Eye, and Dental
Examination
A student, regardless of grade level, who has never been in a Georgia
public school must also provide certification of eye, ear and dental
examinations. Any child admitted to school without a certificate must
present one within 120 calendar days. The form for this examination is DHR
Form 3300. All DHR forms may be obtained at the local public health
departments or physician offices.
Guardianship
If a student resides with any person other than a natural parent, a
certified copy of the court documents that establish legal guardianship
will be required at the time of enrollment. A petition for temporary
guardianship can be filed with the Henry County Probate Court
(770-954-2303).
Immunization
All students in grades K-12 must provide an immunization certificate
(Georgia Form 3231) at the time of enrollment. This certificate may be
obtained from the county health department or from a physician.
A child entering school in Georgia for the first time
must be age-appropriately immunized with all of the required vaccines
(DTP, DTaP, DT, or Td; Hepatitis B; Hib; OPV or IPV; Measles, Mumps and
Rubella or MMR; and Varicella).
For entrance into the sixth grade of
Georgia schools, a child must have at least one additional dose of MMR
vaccine, for a total of two MMR vaccines administered on or after the
child’s first birthday and at least 30 days apart. Also, students entering sixth
grade must be immunized for protection against Chicken
Pox, also known as varicella.
Parents may be able to temporarily waive Georgia's immunization
requirements for religious or medical reasons. Contact the school for
additional information.
Social Security Number
All students entering this school system are asked to provide to the
school the student’s social security number. No student will be denied
enrollment in Henry County Schools for declining to provide his
or her social security number or for declining to apply for such a number.
Proof of Residency

Proof of residence is required when a student
initially enrolls in a school and whenever a change of residence occurs.
To be enrolled in Henry County Schools, students must reside full time in Henry
County with their natural parent(s), or legal guardian(s). Students and
their parent(s)/guardian(s) must remain full-time Henry County residents for
the entire period of enrollment in Henry County Schools. For the purpose
of this policy, a resident is defined as an individual who is a full-time
occupant of a dwelling located in Henry County and who, on any given school
day, is likely to be at their stated address when not at work or school.
A person who owns property in the county, but does not reside in the county, is
not considered a resident for the purpose of this policy.

Transfer Students
Prior to admission, a transfer student shall provide a certified copy of
his or her academic transcript and disciplinary record from the school
previously attended. A student may be admitted conditionally if the name
and address of the school last attended and authorization to release all
academic and disciplinary records to the school is
provided.
Please Note: Any student who is under suspension or expulsion in
another school system and who wants to enroll in Henry County Schools must
clear that suspension/expulsion before any consideration will be given to
the student’s request for approval to enroll.
